CN Guidance & Counseling Services, Inc. is seeking a Care Coordinator to join the CTI team.

The role focuses on delivering time-limited, phase-based care management, developing person-centered plans, linking clients to resources, and supporting skill-building. Ideal candidates hold a Bachelor’s or Master’s in Social Work, Psychology, Education, or related fields, with experience in mental health and co-occurring disorders.

Qualifications

• Bachelor's or Master's in Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology.
• Degrees in other related areas may be considered.

Responsabilités

• Conduct initial and ongoing assessments of assigned clients to document strengths, needs, goals and resources.
• Develop and update client-centered, integrated care plans in consultation with the CTI team to ensure outcomes.
• Maintain communications with clients, primary care physicians, mental health providers, families and agency staff.
• Maintain organized records, statistics and reports per policy and procedure.
• Coordinate care planning with other providers and support transitions.
• Participate in CTI team meetings to review psychosocial conditions and update plans.
