Job summary

Family Residences and Essential Enterprises, Inc. (FREE) is seeking a part-time Registered Nurse (RN) for the CTI and TRS programs. The role blends health services with CTI care management to support adults transitioning from hospitals or crisis settings into the community.

The RN will work across CTI and TRS settings, coordinating care with mental health professionals, care managers, and community providers to promote recovery and independence. Travel to Nassau/Suffolk service areas is required.

Qualifications

• Education requirements include an associate degree in nursing; bachelor’s degree preferred.
• Minimum of two years of RN experience; community mental health experience preferred.
• NYS RN license required; CTI certification within 12 months; 9.58 designation within 6 months.
• Knowledge of behavioral health, physical health, and integrated care; hospital-to-community transitions.
• Excellent communication, documentation, and organizational skills.

Responsibilities

• Provides CTI services to a caseload of adults across engagement, linkage, monitoring, and transfer.
• Conducts health assessments and monitors physical health needs; addresses urgent medical issues.
• Accompanies individuals to medical appointments and ensures follow-up adherence to treatment.
• Maintains medication lists and promotes adherence to schedules; coordinates care.
• Completes documentation per CTI guidance and agency policies; maintains compliance.
• Collaborates with the CTI/TRS team and community providers to support transitions of care.
