Job summary

Housing Forward in Oak Park, IL is seeking a Safe Ride Case Manager to triage, intake, assess, and plan housing solutions for individuals referred to Anderson Shelter. You will document services in HMIS and Salesforce and coordinate with Safe Ride Outreach Specialists and the CTA-sponsored program.

Qualifications

• Bachelor's degree in social work or related field required.
• Master's degree preferred.
• Two years of relevant experience may be considered in lieu of a degree.

Responsibilities

• Coordinate Safe Ride referrals, intake and assessment activities.
• Provide case management and housing planning services.
• Maintain accurate documentation in HMIS and Salesforce.
• Communicate bed availability and client progress to partners.
• Engage in coordination with Safe Ride Specialists and related teams.
