Job summary

Housing Forward is seeking a Safe Ride Case Manager to triage, assess, and plan for housing and services for individuals connected to Anderson Shelter. The role collaborates with Safe Ride Outreach Specialists and CTA partners to maximize bed availability and coordinate placements within transitional shelters or permanent housing options.

The position requires a Bachelor's degree in social work or a related field, with a Master’s preferred, and at least two years of relevant experience.

Qualifications

• Bachelor's degree in social work or a related field is required.
• Master's degree is preferred.
• Minimum of two years of relevant experience may be considered in lieu of a degree.
• Bilingual English/Spanish preferred.

Responsibilities

• Coordinate Safe Ride referrals, complete intake and assessment activities, and provide case management and housing planning.
• Maintain accurate documentation and communicate bed availability and client progress to partners.
• Assist in intake, engagement, and diversion activities, including warm handoffs and bed waitlists.
• Document client services in HMIS and Salesforce.
• Participate in meetings with Safe Ride Specialists to coordinate services for referred clients.
