Job summary

Responsibilities

• Demonstrate excellent customer service consistent with the organization’s Vision, Mission, and Values.
• Manage a caseload and coordinate case management services to meet contract standards; connect to treatment and housing resources.
• Provide supportive counseling and participate in crisis intervention and linkage to crisis teams and hospitals; coordinate crisis plans with psychiatric providers.
• Document clinical records accurately; track referrals, assessments and housing placement plans per policy.
• Make collaborative outreach attempts and arrange transportation to providers for coordinated care and housing solutions.
