Qualifications
Two years of direct case management or supportive services experience working with homeless or disenfranchised individuals.
Knowledge of mental health issues and treatment of substance abuse.
Valid Driver's License and clean traffic abstract.
Responsibilities
Assessment, service plan development and referrals for guests.
Manage a caseload ensuring positive outcomes through timely coordination.
Provide supportive counseling and crisis intervention services.
Document clinical records and maintain assessments and placement plans.
Make collaborative outreach attempts as necessary.
