Job summary

Qualifications

• Bachelor's degree in Human Services, Psychology, Social Work, or related field.
• CASAC certification required or trainee status.
• Minimum 3 years' experience with mental health and/or substance use disorders.

Responsibilities

• Conduct initial and ongoing assessments of residents' mental health, substance use history, and functional capacity.
• Develop and review individualized service plans focused on recovery, housing retention, and personal goals.
• Provide short-term counseling and crisis intervention related to substance use and mental health.
• Educate residents and staff on relapse prevention, harm reduction, and recovery models.
• Link residents to long-term behavioral health services, including outpatient therapy, psychiatric care, and substance use treatment as needed.
• Maintain accurate and confidential documentation including progress notes, assessments, and service plans.
• Facilitate support groups focused on wellness, recovery, and independent living skills.
• Assist residents with life skills development such as budgeting, medication management, and navigating community resources.
• Conduct periodic home visits to assess living conditions and provide in-home support.
• Maintain a small caseload of residents and participate in interdisciplinary team meetings and case conferences.
