Qualifications
Two years of direct case management or supportive services experience working with homeless or disenfranchised individuals.
Valid Driver's License and clean traffic abstract.
Responsibilities
Manage a caseload and ensure positive outcomes through timely coordination of case management services.
Provide supportive counseling and participate in crisis intervention services.
Timely document clinical records and maintain records of referrals and assessments.
