Qualifications

• Two years of direct case management or supportive services experience working with homeless or disenfranchised individuals.
• Valid Driver's License and clean traffic abstract.

Responsibilities

• Manage a caseload and ensure positive outcomes through timely coordination of case management services.
• Provide supportive counseling and participate in crisis intervention services.
• Timely document clinical records and maintain records of referrals and assessments.
