Job summary

Ayden Healthcare Waterville is seeking a dedicated Occupational Therapist to join our team, providing rehabilitation services in skilled nursing facilities and acute care settings between Waterville and Wauseon. The role focuses on developing individualized care plans to help patients regain independence.

You will collaborate with multidisciplinary teams, educate patients and families, document progress, and support discharge planning.

Qualifications

• Experience in occupational therapy within skilled nursing facilities or acute care settings.
• Ability to develop care plans and discharge planning processes.
• Knowledge of physiology and its application in therapeutic practices.

Responsibilities

• Assess patients' needs and develop tailored treatment plans based on individual goals and capabilities.
• Implement therapeutic interventions to improve patients' physical, cognitive, and emotional functioning.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care and effective discharge planning.
• Educate patients and their families on therapeutic techniques and strategies for improving daily living activities.
• Document patient progress and modify care plans as necessary to achieve optimal outcomes.
• Provide support in skilled nursing facilities, focusing on rehabilitation for individuals with varying disabilities.
• Engage in critical care settings to assist patients in regaining independence following acute medical events.
• Work closely with schools to support children with special needs, ensuring they receive appropriate occupational therapy services.
