Job summary

Ayden Healthcare of Waterville and Wauseon seeks an Occupational Therapist to join our team. The OT will develop individualized care plans and implement rehabilitation interventions for patients in skilled nursing facilities and acute care settings.

Responsibilities include assessing needs, collaborating with a multidisciplinary team, educating patients and families, and supporting discharge planning to maximize independence. Travel between Waterville and Wauseon facilities may be required.

Qualifications

• Proven experience in occupational therapy within skilled nursing facilities or acute care settings.
• Ability to develop and implement individualized care plans.
• Knowledge of discharge planning processes for diverse patient populations.
• Strong communication with patients, families and healthcare teams.
• Experience working with individuals with disabilities and varying needs.

We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will have experience working in skilled nursing facilities. This role involves developing and implementing individualized care plans to assist patients in achieving their rehabilitation goals.

Position will float between Ayden Healthcare of Waterville and Ayden Healthcare of Wauseon.

Duties

• Assess patients' needs and develop tailored treatment plans based on individual goals and capabilities.

• Collaborate with multidisciplinary teams to ensure comprehensive patient care and effective discharge planning.

Experience

• Proven experience in occupational therapy within skilled nursing facilities or acute care settings is preferred.
• Familiarity with developing care plans and discharge planning processes for diverse patient populations.
• Knowledge of physiology and its application in therapeutic practices is essential.
• Experience working with individuals with disabilities, including autism spectrum disorders, is highly desirable.
• Strong communication skills to effectively interact with patients, families, and healthcare professionals are required.
