Responsibilities

• Delivery of nursing practices according to evidence-based research, policies, and standards of care to ensure safe, individualized patient care.
• Ongoing communication with patients, nursing colleagues, and health care team regarding patient status and treatment plans.
• Medication administration using bar-scanning and documentation technologies for safe delivery.
• Real-time Point-of-Care electronic documentation of assessments, plans of care, interventions and evaluations.
• Patient and family education to optimize health, wellness, and disease management.
• Multidisciplinary discharge planning to ensure safe transition and continuity of care.
