Responsibilities

• Assessment: admission and ongoing assessment of patient condition and needs.
• Planning: design and document plan of care and discharge goals.
• Implementation: prepare patients for treatments and procedures; ensure patient safety.
• Evaluation: reassess and modify care plans as needed.
• Report Skills: provide bedding shift reports for continuity of care.
