Zepf Center, a longstanding behavioral health provider in Lucas County, Ohio, seeks a full-time Crisis Triage Case Manager for its Emergency Services department in Toledo. The role supports adults, children, and adolescents in crisis, coordinating intake, outreach, and care plans within a 24/7 program.

Hours are 40 per week, Monday through Friday, with no weekends or holidays. The position requires a bachelor’s degree or equivalent mental health experience, valid Ohio driver’s license, and

Qualifications

• Must be eligible to be a qualified mental health specialist by Ohio Medicaid.
• Bachelor's degree in a related field OR at least three years' work experience in mental health.
• Must possess a valid Ohio driver’s license.

Responsabilités

• Gathers collateral data at the scene or by phone to resolve crises.
• Provides outreach crisis intervention.
• Collaborates with patient’s psychiatrist/provider, case manager and/or counselor.
• Facilitates client care in consultation with health professionals and providers.
• Makes phone calls and coordinates services; documents in client charts.
