Job summary

The Shelter Association of Washtenaw County (SAWC) seeks a Crisis Stabilization Case Manager to coordinate outreach with Washtenaw County Community Mental Health and oversee housing stabilization services for seven-bed housing programs.

You will engage clients, develop Individual Service Plans, arrange referrals, and work with partners to secure safe, affordable housing using Housing First principles. The role requires daytime hours with some evenings and weekends.

Qualifications

• Bachelor’s Degree or equivalent experience; knowledge of mental health and housing issues is helpful.
• Willingness to adhere to NASW Code of Ethics.

Responsibilities

• Assist with the orderly running of the Delonis Center.
• Manage caseload of active cases not primarily served in residential program.
• Meet with clients in the community and provide referrals.
• Maintain client records and HMIS data entries.
• Work weekends/evenings during winter months and holidays as assigned.
• Be on call to respond to referrals until 9 pm, Mon-Fri.
