Job summary
A community mental health center in Leavenworth is seeking a Crisis Stabilization Case Manager to provide essential case management services to adult clients in crisis. This role involves coordinating communication and services across internal and external partners while providing direct care aimed at stabilizing mental health and substance use symptoms. Candidates should hold a Bachelor’s degree in Social Work, Psychology, or a related field, and have experience working with individuals in the community. Evening and weekend availability is required.
Qualifications
Bachelor’s degree required. Degree in Social Work, Psychology, or Human Service field preferred.
Equivalent work experience in the human services field will be considered.
One year volunteer or paid experience working with individuals in the community.
Responsibilities
Gather client information and refer them to appropriate resources.
Provide support to other providers in crisis interventions.
Make follow-up calls to clients after crisis.
Assist in coordinating access to appropriate care after evaluation.
Collaborate with team members and outside providers.
Help maintain daily operations of the Crisis Stabilization Center.
Support staff members as needed.
Assess and utilize de-escalation skills in intense situations.
Maintain professional communication with TGC staff and partners.
Adapt to changing work priorities in a fast-paced environment.
