Job summary

Shelter Association of Washtenaw County (SAWC) is seeking a Crisis Stabilization Case Manager in Ann Arbor, MI to lead outreach with WCCMH and coordinate housing stabilization for 7-bed shelter clients. This role requires ISP development, housing plans, and close collaboration with county partners.

The position is salaried and exempt, with a 48,960 USD base salary and Union representation. The role supports vulnerable populations, including those experiencing homelessness, with a flexible

Qualifications

• Bachelor’s Degree or equivalent experience; knowledge of mental health, housing, or related social services helpful.
• Excellent verbal and written communication skills.
• Commitment to diversity and ethical standards.

Responsibilities

• Coordinate and conduct outreach with Washtenaw County Community Mental Health (WCCMH).
• Orient admitted clients into the Housing Crisis Stabilization (HCS) program.
• Create Individual Service Plans (ISP) with clients to stabilize mental health and secure housing.
• Maintain accurate client records and ensure program compliance.
• Work flexible shifts, including evenings and weekends as needed.
