Job summary

Bay Area Community Services (BACS) is seeking Clinical Care Coordinators for the new CWRT in Sacramento. The team operates 24/7 with field responders deescalating crises and connecting clients to resources, collaborating with 911/988, county dispatch, and law enforcement.

Responsibilities include crisis response, service planning, counseling, and case management, with travel across the region. A MSW/MFT/MPCC and active California licensure track are required, plus travel and vehicle use.

Qualifications

• Master's degree in social work, marriage and family therapy, or clinical counseling.
• Registered with the BBS as an Associate ASW/LCSW or Associate MFT/LMFT/APCC.
• 2 years of social service experience required.
• Bilingual in a threshold language preferred.
• Valid California driver’s license and personal vehicle.
• Extensive local travel required; DMV printout required.

Responsibilities

• Dispatching and response for crises referred by 911/988 for alternative mobile crisis response.
• Work closely between mobile teams, dispatch and county law enforcement, 911 or 988 to deescalate crisis situations.
• Provide individual, family, group counseling, and case management for individuals in the field.
• Utilize Evidence-Based Practice to assure high-quality results and impact.
• Assess risk status and intervene appropriately.
• Create service plans focused on client recovery and goals.
• Outreach, engagement, and reporting disengagement to the team.
• Documentation and data collection in accordance with policies.
