Cascade Community Healthcare in Centralia, WA is seeking a Substance Use Disorder Professional to join the Crisis Relief Team. You will assess, diagnose, and plan treatment for adults in crisis, coordinating detox services with an interdisciplinary team.

This role requires a related degree, WA SUDP credential, and knowledge of ASAM criteria; strong teamwork and clear communication are essential. 0.9 FTE, benefits and supportive program offerings included.

Qualifications

• Associate degree in human services or related field; bachelor’s preferred.
• WA SUDP credential required.
• Working knowledge of ASAM criteria and placement guidelines.

Responsibilities

• Conduct Substance Use Disorder Assessments in accordance with ASAM and clinic policies.
• Educate individuals on treatment consents and authorizations for use and disclosures.
• Participate in interdisciplinary treatment team meetings.
• Develop and implement individualized, person-centered treatment plans and review with involved parties.
• Coordinate care and provide referrals to ensure seamless service delivery.
• Identify gaps in service needs and recommend discharge plans with the team.
• Maintain safety and document activities in the EHR according to best practices.
• Ensure compliance with regulatory, contractual, and legal requirements.
