Job summary

Talentify is seeking a Case Manager (CM) in Philadelphia to join a multidisciplinary team delivering behavioral health services. The CM will help design and implement treatment plans, participate in crisis management, and provide direct oversight for Members while guiding transitions to less structured living arrangements.

Responsibilities include admissions collaboration, incident reporting, weekly CM sessions, group facilitation, on-site crisis support, daily living assistance, and discharge

Qualifications

• Must have at least a high school diploma with 3 years of experience in mental health, behavioral health, or drug and alcohol field.
• Or an associates degree with 2+ years of experience in the same fields.
• Must possess a valid, non-commercial driver's license with a clean driving record.
• Excellent communication, interpersonal, organizational and time management skills.
• Demonstrated ability to work with a multidisciplinary team.

Responsibilities

• Develop treatment plans with the multi-disciplinary team and monitor progress.
• Complete weekly individual Case Management sessions and document in DAP format.
• Collaborate with admissions to support intake and acclimation of new Members.
• Complete incident reports per MCO/State/Policy requirements.
• Provide crisis management support and on-call coverage as needed.
• Assist with daily living activities and transportation to appointments.
• Support discharge planning and linkage to community services.
