Job summary

Connections Health Solutions in Phoenix, AZ is seeking a Registered Nurse to assess consumers and document triage, behavioral and physical health issues within a crisis-care setting.

You’ll work as part of a multidisciplinary team, perform nursing assessments, plan care, provide education to patients and families, and help de-escalate potentially dangerous situations.

Qualifications

• Graduated from an accredited Nursing program.
• Unrestricted active RN license in Arizona or multi-state licensure.
• Must meet company vaccination policy requirements.

Responsibilities

• Completes nursing assessments including Triage, Medical/Behavioral Health assessments, and medication administration.
• Plans individual treatment as part of the Multidisciplinary Clinical team.
• Provides education regarding psychiatric diagnosis, treatment options, psychopharmacology, and health literacy to patients and families.
• Intervenes with verbal de-escalation to diffuse potentially dangerous situations and documents outcomes.
• Adheres to Seclusion/Restraint standards, initiates documentation and debriefing as required.
• Transcribes provider orders, administers medications, and performs chart checks.
• Performs discharge nursing tasks and other duties as assigned.

Education

Bachelor's degree in Nursing

Unrestricted active RN license
