Job summary

Connections Health Solutions in Phoenix, AZ is hiring a Registered Nurse (RN) to assess consumers and document triage, behavioral and physical health issues within a crisis-care setting. You will work with a multidisciplinary team, plan individual treatment, educate patients and families about diagnoses and medications, and apply crisis-intervention techniques to de-escalate difficult situations.

Qualifications

• Graduate from an accredited nursing program.
• Unrestricted active RN license in Arizona.
• Mandatory vaccination policy compliance or approved exemption.
• Ability to perform physical interventions and lift up to 50 lbs.

Responsibilities

• Conduct nursing assessments and document triage, medical and behavioral health issues.
• Plan treatment as part of the multidisciplinary team.
• Educate patients and families on diagnosis, medications, and care options.
• Intervene verbally to de-escalate potentially dangerous situations.
• Follow seclusion/restraint guidelines and document debriefings as required.
• Transcribe orders and administer medications per physician directives.
• Perform discharge nursing checks and chart reviews.
