Qualifications

• 6 months of experience working with adults who have mental illness preferred.
• Ability to work independently in a community-based setting.
• Valid Colorado Driver’s license with a clean driving record.

Responsibilities

• Document interventions and patient follow-up in EHR system.
• Facilitate consumer empowerment and goal achievement.
