Job summary

Community Reach Center in Thornton, Colorado is seeking a Crisis Navigator to provide clinical and support services within integrated behavioral health care. You will help consumers and families develop skills, coordinate with care teams, and connect them to ongoing treatment in a 24/7 resource environment.

The role emphasizes brief interventions, documentation in the EHR, and collaboration with emergency services to ensure safe transitions to higher levels of care.

Qualifications

  • Bachelor's degree in psychology or related field.
  • 6 months of experience with adults who have mental illness preferred.
  • Valid Colorado driver's license with clean driving record.

Responsibilities

  • Provide prevention/early intervention, care navigation and family psychoeducation.
  • Coordinate mental health care with the consumer's care team and family.
  • Deliver brief interventions using evidence-based techniques (e.g., motivational interviewing).
  • Document encounters in the EHR and track follow-up and outcomes.
  • Attend treatment team meetings and participate in reviews.
  • Provide crisis intervention and emergency services as needed.
  • Assist consumers in gaining access to medical, social, educational and other services.
  • Develop service plans, referrals, linkage, and follow-ups.

About the Role:

The Crisis Navigator “CN” provides clinical and support services to consumers and/or their families through integrated behavioral healthcare services and resource navigation. The Crisis Navigator “CN” will assist the individual to explore and develop social, occupational, leisure, and living skills, as well as linking to ongoing treatment in the continuum of care. The Resource and Walk‑in Crisis Center provides 24/7 support to individuals who are experiencing a mental health crisis, or, looking for resources to prevent a crisis. This position collaborates with emergency services staff to provide referral and transition services to consumers needing a higher or different level of care. We are seeking professional and compassionate individuals to support clients in crisis and to provide and/or arrange for therapeutic interventions as appropriate for consumers in a crisis/emergency condition.

Essential Functions:

  • Provide prevention/early intervention, care navigation, family psychoeducation, and other clinical interventions to consumers and families as appropriate to the position.
  • Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider and family members as needed.
  • Provide brief interventions using evidence‑based techniques such as Motivational Interviewing, de‑escalation, or other treatments appropriate to support consumer engagement.
  • Document initial intervention, track patient follow‑up, and clinical outcomes using an Electronic Health Record system (EHR). Document in‑person and telephone encounters in the EHR, and use the system to identify and re‑engage patients.
  • Attend collaborative treatment team meetings and reviews of consumers
  • Provide therapeutic crisis intervention and emergency services as needed
  • Assist eligible consumers in gaining access to needed medical, social, educational, and other services.
  • Provide an assessment of needs, service plan development, referrals/linkage, and monitoring/followups for consumers.
  • Facilitates and contributes to the accomplishment of tasks by consumers, empowers consumers to reach optimum level of achievement, and sets a positive example and provides role modeling, as well as technical expertise, to accomplish goals.
  • Attend and participate in group sessions, in‑service education and training opportunities, meetings, conferences,and special events in order to utilize opportunities for professional growth.
  • Provide transportation for consumers in agency vehicles when clinically indicated.
  • Work closely with outside organizations on resources and care coordination.
  • Uses identified agency key performance indicators to track success, engagement, and other factors leading to successful completion of treatment.

Qualifications:

  • Requires bachelor’s degree in psychology, Social Work, or directly related field
  • 6 months of experience working with adults who have mental illness, particularly those individuals diagnosed with a serious and persistent mental illness preferred but not required.
  • Communication, organization, time management and clinical skills.
  • Ability to work independently in a community‑based setting.
  • Valid Colorado Driver’s license with a clean driving record.

PRN

Salary Information:

$24.50 to $28.00 per hour

Eligible for $250/month Bilingual Stipend
