Qualifications

• RN or MSW/counseling degree required and Ohio license orlicensure within 90 days of hire.
• Minimum of one year of case management or psychiatric experience.
• Ability to interact with patients and families in crisis with confidentiality and professionalism.

Responsibilities

• Provide emotional support, crisis intervention, and discharge planning.
• Assess and develop an appropriate plan for continuity of care for patients and families.
• Complete comprehensive assessments, interventions, and documentation in the medical record.
• Explain and complete Advance Directives with patients and families.
• Provide crisis stabilization, evaluations, intakes, and hospital pre-screening in multiple Memorial Health System locations.
• Facilitate psychiatric hospitalizations when needed and coordinate alternative plans of care.
• Provide collateral contacts to ensure high-quality care for clients.
• On-call responsibilities may be required.
