Job summary

Great River Health Systems is seeking a Crisis Interventionist to provide direct support to patients with mental health needs. You will perform psychosocial screenings, de-escalation, and crisis stabilization while coordinating care and facilitating referrals to community resources, including tele-medicine assessments.

The role involves working within the Behavioral Health Assessment Team, conducting brief intervention sessions, and documenting progress notes accurately for timely disposition

Qualifications

• Master’s degree in social work or related field required.
• 4–6 years experience in mental health/social work settings.
• MOAB training within 1 year of hire.
• Knowledge of psychosocial assessment and treatment principles.

Responsibilities

• Provide crisis stabilization and de-escalation for patients.
• Conduct psychosocial screenings and triage for patients in ED or other departments.
• Coordinate care and connect patients with community resources.
• Conduct brief intervention counseling sessions and document progress.
• Refer to resources for housing, legal aid, financial assistance, etc.
