Job summary

Claratel Behavioral Health is seeking a Case Manager (Paraprofessional) to support clients across PES and CSU. You will handle intake, triage, and treatment coordination, working with a multidisciplinary team to assess needs, coordinate services, and facilitate groups.

The role emphasizes trauma-informed, recovery-oriented, client-centered approaches, discharge planning, and connection to community resources. Bachelor’s degree required; MSW preferred.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field required.
• MSW or Counseling degree preferred; crisis stabilization experience is a plus.

Responsibilities

• Greet, triage, and intake with PES/CSU clients, collecting demographics and insurance.
• Update treatment plans, provide 1:1 support, and assist with discharge planning.
• Facilitate psychoeducational groups and document participation and outcomes.
• Coordinate referrals, transportation, and follow-up with community resources.
