Job summary

Responsibilities

• Intake & triage for PES, collecting demographics and insurance info, recording vital signs and safety checks.
• Ongoing support and monitoring in CSU and PES, updating treatment plans and providing 1:1 support.
• Treatment & group facilitation in CSU, including psychoeducational and recovery-oriented groups.
• Discharge planning and care coordination, including arranging placements and follow-up appointments.
• Administration & compliance, including accurate documentation and incident reporting.
