The Archdiocese of St. Louis seeks a Case Manager - Generalist to provide comprehensive care for individuals and families in crisis or homelessness.

You will use assessments to address basic needs, stabilize crises, and help create long-term recovery plans. This role involves coordinating services across counties, advocating for housing, budgeting education, and ensuring timely case documentation while maintaining confidentiality.

Qualifications

• 2-3 years of related work experience required.
• 1-3 years supervisory experience required.

Responsibilities

• Act as the Service Advocate for those in crisis.
• Coordinate uniform certification and assessment across three counties.
• Refer to specialized agencies for complex needs.
• Advocate with specialized agencies to address complex needs.
• Work in a team to manage client care.
• Utilize assessment tools and service plans as needed.
• Ensure all client interactions are entered into the system in a timely manner.
• Develop and review service plans with clients; follow up on outcomes.
• Assist housing stability through contact with landlords/management.
• Provide tenant counseling on leases and utilities; mediator with landlords as needed.
• Assist with budgeting and credit issues.
• Conduct community outreach to raise awareness on homelessness.
• Monitor contracted and pledged services for compliance.
• Attend staff meetings and trainings.
