Qualifications
Understanding of mental illness and substance use.
Trauma Informed Care Trained.
Experience with wrap-around services preferred.
Requires reliable vehicle and valid auto insurance.
Responsibilities
Conduct comprehensive assessments of mental illness and other relevant needs.
Provide direct clinical services to clients on individual and group basis.
Refer clients to appropriate community agencies.
Collaborate with other members of the case management team.
Ensure the care model by interacting with all other St. Patrick Center programs.
