Catholic Charities of the Archdiocese of St. Louis, St. Patrick Center, is seeking a Case Manager for the Housing, Employment and Recovery Opportunities (HERO) program.
The case manager will meet with clients in transitional housing, coordinate services, document support, and track ISS/Treatment Plan of Care to help clients achieve stable housing and financial independence. Starting compensation is $45,000.
St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability.
Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.

The case manager will meet with clients to maintain family and natural supports in the community, collaborate with clients and track the success of the Individual Service Strategy (ISS)/Treatment Plan of Care, coordinate individualized services and the involvement of providers working in the home and community setting and document all support related to activities and services. Starting compensation is $45,000
