Job summary

Catholic Charities of the Archdiocese of Chicago is seeking a Case Manager - Housing Stability to determine client eligibility, perform assessments, and develop service plans that connect clients to housing resources and supportive services.

The role requires collaboration with multidisciplinary teams, accurate documentation, and advocacy to help clients achieve stability and self-sufficiency. A BA/BS with 1–3 years’ experience is expected, with LSW/LCSW preferred; Spanish proficiency is a plus.

Qualifications

• Bachelor's Degree in Arts/Sciences with 1-3 years relevant experience.
• Excellent written and verbal communication skills.
• Strong interpersonal skills.
• Ability to work effectively both independently and as part of a team.
• Proficiency in Microsoft Office suite.

Responsibilities

• Assess prospective clients for program eligibility and conduct housing inspections.
• Develop, implement, and monitor individualized service plans and connect clients to resources.
• Document client interactions, progress, and outcomes per CCAC policies and regulatory requirements.
• Collaborate with multidisciplinary teams and community partners to coordinate services.
• Provide emotional support and guidance to clients.
