Job summary

Oaks Integrated Care Inc. is seeking a Case Manager to support adults in an Emergency Room Alternative program focused on wellness, recovery, and coping skills.

You will work to prevent hospitalization and connect clients to community resources, conducting initial assessments and referrals as needed. Qualifications

• BA/BS with at least 1 year in the mental health system or MA/MS preferred.
• Clinical training in group work and working with families.
• Knowledge of mentally ill in imminent distress.
• Knowledge of NJ mental health field and recovery principles.

Responsibilities

• Manage a caseload of 30-40 individuals providing short-term crisis case management.
• Follow-up with people experiencing a psychiatric emergency to provide an alternative to hospitalization.
• Link consumers to other mental health settings with quick turnaround.
• Meet with walk-ins to complete initial documentation and assess referral.
