Job summary

Oaks Integrated Care is seeking a Case Manager to work in our Integrated Case Management Program across Southern New Jersey. You will provide intensive, community-based services to adults with severe mental illness, coordinating care, advocating for resources, and supporting independence and recovery.

The role includes on-going collaboration with the clinical team, rotating on-call crisis coverage, transportation support, and travel within Southern New Jersey.

Qualifications

• MSW or MA/MS with clinical training is required.
• Bachelor's in Psychology, Social Work or Mental Health Counseling plus 1 year relevant experience.
• Valid driver's license in good standing; pre-employment drug screening.

Responsibilities

• Provide intensive community-based services to seriously and persistently mentally ill individuals in the community.
• Facilitate service linkage across health systems; coordinate and integrate services from multiple providers.
• Provide direct service support including transportation, treatment team coordination, and daily living skills.
• Maintain an electronic consumer clinical record reflecting all services provided on time.
• Complete Daily Activities in the Electronic Clinical Record.
• Provide 24/7 on-call crisis intervention on a rotating basis, including evenings and weekends.
• Collaborate with the Clinical Team and travel within Southern New Jersey.
