CCEOK seeks a full-time Case Manager in Tulsa, OK, to provide compassionate, strengths-based case management for individuals and families facing crisis and economic hardship. You will assess needs, develop service plans, and navigate resources to promote long-term stability and self-sufficiency.

You will work within a Catholic framework, coordinating with the Household Stability team and community partners, and engaging clients through motivational interviewing and client-centered planning.

Qualifications

• Master's degree in social work or related field preferred; bachelor’s degree or equivalent experience required.
• Minimum three years of experience in case management or related human services field preferred.
• Bilingual in English and Spanish preferred.
• Ability to articulate Catholic teaching and integrate Catholic Social Teaching into service delivery.
• Strong communication, critical thinking, and problem-solving skills.
• Proficiency with Microsoft Office and electronic case management systems.

Responsabilités

• Conduct strengths-based assessments to identify needs and barriers to stability.
• Develop individualized service plans and monitor progress.
• Advocate for clients and connect with housing, employment, healthcare resources.
• Educate clients on budgeting and community resources.
• Maintain documentation and ensure compliance in databases like Blackbaud and Apricot.
• Collaborate with TL team and community partners to coordinate services.
