Endeavors is seeking a dedicated Case Manager to deploy to public emergencies and support clients across crisis situations. You will manage a caseload, conduct needs assessments, and develop individualized plans with a focus on safety and continuity of care.

Responsibilities include coordinating referrals, maintaining documentation, and ensuring compliance with state and federal standards. Travel to multiple locations and proactive client engagement are essential to success.

Qualifikationen

• Bachelor’s degree or undergraduate degree in Social Work, Psychology, Criminal Justice, Sociology or related field.
• 2+ years of crisis intervention experience preferred; proficient in crisis scenarios.
• Proficiency in Spanish/English (written and spoken) required.
• Must complete National Incident Management System (NIMS) Core Curriculum Courses.

Aufgaben

• Coordinate referrals, service planning and documentation for assigned caseload.
• Interview applicants to determine eligibility and establish program goals.
• Develop client intake and case plans; complete necessary paperwork and social history.
• Meet regularly with clients to discuss progress and maintain records.
• Maintain confidentiality and adhere to agency policies and licensing requirements.
• Travel to various locations as needed and provide exceptional customer service.
