The Crisis Care Coordinator at MultiCare Health System in Lakewood, WA is responsible for providing follow up services to individuals in crisis and coordinating referrals and warm hand-offs to community services to help stabilize and reduce future crises.

The role collaborates with law enforcement to divert individuals from incarceration or unnecessary emergency visits and requires a WA driver’s license and background checks.

Qualifications

• Bachelor's degree in social sciences, mental health or related field required; Master's degree preferred
• Washington State ACCR credential must be applied for within 30 days of hire or LMHCA credential may be substituted
• Current Washington state driver's license
• Minimum two years of experience preferred
• Proof of a safe driving record via motor vehicle report
• Must be approved and pass additional background checks from correctional facilities and jails that refer to crisis team

Responsabilités

• Provide follow up services for individuals seen in crisis and coordinate referrals and warm hand-offs to appropriate services
• Coordinate care within and across systems to ensure linkage to services
• Collaborate with law enforcement and first responders to educate and divert individuals from incarceration or unnecessary ED visits
