Job summary

MultiCare is seeking a Crisis Care Coordinator in Lakewood, WA to follow up with individuals in crisis and coordinate referrals to community services. You will work with the crisis team to stabilize clients and help divert unnecessary emergency department visits.

The role requires a Bachelor's degree in social sciences or related field, WA ACCR or LMHCA credentials, and a valid WA driver's license, with background checks and potential further facility clearances.

Qualifications

• Bachelor's degree in social sciences, mental health or related field required.
• Washington State Counselor Agency Affiliated Registered (ACCR) credential must be applied for within thirty (30) days of hire. Employees may work while the application is pending for up to 120 days, OR a Mental Health Counselor Associate (LMHCA) credentialed with Washington State may be substituted for the ACCR.
• Current Washington state driver's license
• Minimum two (2) years of experience preferred
• Must pass background checks and obtain clearance from correctional facilities/jails that refer to crisis team

Responsibilities

• Provide follow up services for individuals seen in crisis and coordinate referrals and warm hand offs to appropriate services.
• Assist in gaining access to services and coordinate care within and across systems.
• Conduct risk assessments and collaborate with law enforcement and first responders to educate.
• Work with crisis team and co-responders to divert individuals from incarceration or ER visits.

Education

FTE: 1.0, Shift: 10 hour shifts, daytime, Schedule: Monday through Thursday.

Candidates must pass a background check with the Lakewood Police department to be offered this position.

Candidates also have to have a valid driver's license in Washington state.

Position Summary

The Crisis Care Coordinator is responsible for providing follow up services for individuals seen in crisis and helping coordinate referrals and warm hand offs to appropriate services in the community to help stabilize and/or mitigate future crisis The incumbent provides direct assistance in gaining access to services; coordinates care within and across systems; ensures linkage to appropriate services; conducts assessment of the eligible recipient to determine service needs; evaluates and completes a risk assessment; collaborates with law enforcement and other first responders to educate; works closely with all member of the crisis team and co-responders to help divert individuals from incarceration or unnecessary visits to the emergency department.

Requirements

Bachelor's degree in social sciences, mental health or related field required; Master's degree preferred

Washington State Counselor Agency Affiliated Registered (ACCR) credential must be applied for within thirty (30) days of hire. Employees may work while the application is pending for up to one hundred twenty (120) days, OR a Mental Health Counselor Assoc (LMHCA) credentialed with Washington State may be substituted for the AAC

Current Washington state driver's license

Minimum two (2) years of experience preferred

Proof of a safe driving record (which meets established MultiCare Standards) obtained by a motor vehicle report from the appropriate state

Must be approved, have clearance, and pass additional background checks from correctional facilities and jails that make referrals to crisis team and agree to annual background checks

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $30.40 - $43.75 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
