Four Corners Community Behavioral Health is seeking a Case Manager for a full-time day shift (6 a.m. to 6 p.m.) at the Crisis Receiving Center in Price, UT. You will greet clients in crisis and help with intake while ensuring a safe, compassionate environment.

Responsibilities include de-escalation, connecting clients to community resources, coordinating discharge plans with staff, and maintaining a clean, safe center. Must lift 20 lbs and may transport clients as needed.

Description

Four Corners Community Behavioral Health is seeking a Case Manager, full-time, day shift, 6 a.m. to 6 pm. Duties include greeting clients in a crisis setting and assisting in supporting clients’ needs throughout their stay at the Crisis Receiving Center. Assisting clients with intake paperwork, notifying clients of the rules of the center, and ensuring a positive and safe work environment are critical. Assisting with crisis de-escalation by assessing clients’ needs and connecting them to resources within the community are essential functions of the job. Maintaining a clean and safe work environment is also an essential job duty, and completing day-to-day cleaning tasks for the center will be required. Must be able to lift 20 lbs. and bend, sit and stand for periods of time. Communicating discharge plans with clients and staff, as well as day-to-day communication with staff is also required. Some transportation of clients may be required. Candidates must also work with staff to ensure individuals receive the utmost care and compassion throughout their experience. Strong coordination skills and relevant work experience are preferred. 36 to 40 hours/week, depending on applicants’ desires, with an excellent benefits package. Starting pay is $18.57/hr.; salary depends on experience and education. We offer a $2.00/hour differential for shifts worked on Saturdays and Sundays and between the hours of 11 pm to 5 am. Up to an additional $4.00/hr. available!
