Job summary

Compass Behavioral Health in the United States is seeking a full-time Case Manager to provide CPST and Crisis case management. You will deliver goal-directed support and solution-focused interventions, with hours flexible to meet client needs.

Maintain contacts per the treatment plan, coordinate with CM staff, and attend 90-day plan meetings after crises. Progress notes and DLA-20 coordination are required, using the EMR scheduler for updates.

Qualifications

• BA/BS degree in Mental Health or Human Services or equivalent experience.
• Excellent interpersonal and teamwork skills.
• Strong organizational and time management abilities.
• Must have a valid driving license.

Responsibilities

• Provide case management (CPST, Crisis) to assigned caseload.
• Deliver goal-directed, solution-focused interventions.
• Contact new referrals within 24 hours of opening.
• Maintain contacts per the Treatment Plan; coordinate with CM staff when out of office.
• Attend treatment plan meetings every 90 days and after crises.
• Complete DLA-20 every 90 days and coordinate CBCLs.
• Utilize and keep the EMR scheduler updated.
