Qualifications

• Must meet all federal, state, and local requirements.

Responsibilities

• Assist patients with daily living per plan of care: bathing, oral hygiene, dressing, toileting, feeding.
• Take and record vital signs as ordered (temperature, pulse, respiration, blood pressure).
• Assist with safe transfers and ambulation; support exercises per care plan.
• Perform routine tasks such as laundry and patient safety measures.
• Communicate clearly with client, family, and nurse supervisor; document changes in patient condition.
