The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide who speaks creole to assist in patients' homes in accordance with an established care plan.

The role involves helping with daily living activities and personal care per the plan of care, while maintaining safety and clear communication with families and supervisors. Responsibilities include helping with baths, meals, hygiene, transfers, and documentation.

Formación

• CPR certification required (current).
• Read and follow written care plans.
• Complete Maxim competency program.
• Meet federal, state, and local requirements.

Responsabilidades

• Assist with daily living tasks per plan of care.
• Record vital signs as ordered.
• Assist with baths, dressing and hygiene.
• Safe transfer and ambulation according to plan.
• Document changes and report to assigned nurse supervisor.
• Communicate clearly with client, family and office.

The Lakes Home Care is looking for a compassionate Home Health Aide who speaks creole to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

Responsibilities

• Responsible for assisting patients with and per-plan of care, the following:

  • Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc.

  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)

  • Safe transfer and ambulation techniques

  • Exercise according to the plan of care

  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)

  • Patient’s laundry

  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)

  • Observation and teaching of home safety practices

  • Recognition of an emergency situation and knowledge of emergency management procedures

• Must follow patients’ plan of care and complete all tasks assigned

• Communicates in a constant, clear, and effective way with the client, family/caregiver and office

• Observes and documents patient condition and reports any changes, to the assigned nurse supervisor

Requirements

• Meets all training requirements in accordance with state and federal laws when applicable

• Ability to read, follow written instructions and document care given

• Possess and maintains current CPR certification in accordance with state regulation

• PPD or chest x-ray if applicable

• Successful completion of Maxim screening examinations

• Participation and completion of Maxim’s competency program
