HealthPartners is seeking a Counselor for the Amery Program for Change to support patients in intensive outpatient, outpatient, and continuing care. The AODA Counselor will help patients reach treatment goals and participate in curriculum planning within a collaborative care setting.
The role emphasizes excellent organization, case management, and communication with families and health professionals, with opportunities to apply EPIC EHR and office software skills in daily practice.

The AODA Counselor is a member of our Programs for Change team and facilitates treatment environments for patients to obtain the knowledge and tools necessary to complete all aspects of intensive outpatient, outpatient, and continuing care treatment plans and individual goals of treatment.
