Hudson Hospital & Clinic, part of HealthPartners, is seeking a Substance Abuse Counselor to support patients in intensive outpatient and continuing care plans, and to help individuals meet their treatment goals. The role includes completing intake assessments and coordinating care with the treatment team.
The position requires an Associate's degree in a SAC-related program and eligibility for Wisconsin SAC licensure, with preferred Bachelor's degree and broader licensure.
The Substance Abuse Counselor facilitates patients in obtaining the knowledge and tools to complete all aspects of intensive outpatient and continuing care treatment plans and individual goals of treatment. Complete all aspects of the intake assessment process.
