Job summary

Nexus Health seeks a Certified Occupational Therapist Assistant (COTA) to work under an Occupational Therapist in a facility setting, delivering rehabilitative services to residents to improve daily living activities.

You will implement treatment plans, educate residents and families, and maintain accurate documentation while collaborating with a care team. Prior SNF/long-term care experience and PCC familiarity are preferred, with a commitment to quality of life and safety.

Qualifications

• Prior experience in a skilled nursing facility (SNF) or long-term care setting preferred.
• Strong knowledge of current treatment practices and therapy interventions.
• Familiarity with PCC or similar electronic documentation systems preferred.
• Ability to manage a diverse patient caseload with professionalism and adaptability.

Responsibilities

• Plan and administer treatment programs for residents and patients to relieve pain, restore or improve function.
• Provide skilled occupational therapy interventions under supervision of the Occupational Therapist.
• Educate residents, families, and staff regarding therapy goals, techniques, and outcomes.
• Demonstrate competent technical skills consistent with COTA standards of practice.
