Job summary

Nexus Health seeks a Certified Occupational Therapist Assistant (COTA) to work under an Occupational Therapist in a facility setting, delivering rehabilitative services to residents to improve daily living activities.

You will implement treatment plans, educate residents and families, and maintain accurate documentation while collaborating with a care team. Prior SNF/long-term care experience and PCC familiarity are preferred, with a commitment to quality of life and safety.

Qualifications

  • Prior experience in a skilled nursing facility (SNF) or long-term care setting preferred.
  • Strong knowledge of current treatment practices and therapy interventions.
  • Familiarity with PCC or similar electronic documentation systems preferred.
  • Ability to manage a diverse patient caseload with professionalism and adaptability.

Certified Occupational Therapist Assistant (COTA)

Position Overview:

Works under the supervision of a licensed Occupational Therapist to provide rehabilitative services that help residents improve or maintain their ability to perform daily living activities. The COTA implements individualized treatment plans to promote independence, safety, and overall quality of life.

Key Responsibilities:

  • Plan and administer treatment programs for residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.

  • Provide skilled occupational therapy interventions in accordance with physician orders and the established plan of care, under the supervision of the supervising Occupational Therapist.

  • Educate residents, families, and staff regarding therapy goals, techniques, and outcomes.

  • Demonstrate competent technical skills consistent with current COTA standards of practice.
