Five Star Nursing is seeking a Certified Occupational Therapy Assistant (COTA) to join our Skilled Nursing Facility team. You will collaborate daily with the OT Supervisor, following established Plans of Care to deliver high-quality therapy services.

The position supports patients with conditions such as TBI, CVA, stroke, Parkinson’s, MS, and orthopedic issues, uses EMR for progress notes, and offers flexible scheduling, weekly pay, and comprehensive benefits for full- or part-time positions in

Qualifications

• Current NYS OTA License and Registration.
• EMR competence.
• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation).
• Valid Proof of Identity and Work Authorization.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of EMR software.

Occupational Therapy Assistant Job Duties:
