CPS Healthcare is seeking a Psychiatric Nurse Care Coordinator to support the Suffolk County House of Corrections in a full-time, Monday–Friday role. The RN will help coordinate, ensure continuity of care, and oversee safe delivery of psychiatric services within the facility.

The successful candidate will work with the Mental Health Director and psychiatric provider to optimize workflow, monitor medications, and ensure timely communication across disciplines.

Qualifications

• Current, unrestricted license as a Registered Nurse.
• Clinical experience caring for individuals with psychiatric and behavioral health conditions.
• Knowledge of psychiatric medications, monitoring, and adherence considerations.
• Ability to coordinate care across multiple disciplines and healthcare settings.

Responsabilités

• Maintain familiarity with active psychiatric caseload and related nursing needs.
• Support development and management of psychiatry clinical workflow with the team.
• Contribute nursing and medication information affecting scheduling and priority.
• Coordinate verification and continuity of psychiatric medications after admissions or transfers.
• Monitor medication-related needs and adherence, and arrange follow-up as needed.
• Gather and verify relevant clinical information to support evaluation and treatment.
• Assist readiness for psychiatric evaluation by addressing outstanding needs.
• Serve as primary nursing contact for routine psychiatric medication concerns.
• Facilitate communication of psychiatric concerns through clinical pathways.
• Coordinate treatment plan follow-through with nursing, mental health, medical, and pharmacy teams.
