Qualifications

• Experience in skilled nursing, long-term care, or a healthcare setting.
• Familiar with referral processes, payers (Medicare/Medicaid), and admission procedures.
• Strong communication and interpersonal skills with professional phone etiquette.
• Proficient with Microsoft Office and electronic referral systems.
• Highly organized with multitasking and prioritization in a fast-paced environment.

Responsibilities

• Receive, review, and respond to referrals promptly and professionally.
• Coordinate with admissions teams to determine bed availability and payer source eligibility.
• Enter referral data into the referral management system accurately.
• Serve as liaison between referral sources and facility teams.
• Monitor referrals and follow up to ensure timely admissions.
• Maintain logs and tracking to document outcomes.
• Provide excellent service to internal teams and external partners.
• Support admissions processes and provide backup coverage as needed.
