Job summary

ViaQuest seeks a Coordinator of Quality Outcomes/Program Director to lead the Circle of Care services. This full-time role includes overseeing the system of supports, coordinating service delivery, and ensuring regulatory standards across sites.

Requirements include a bachelor’s degree or 8–10 years of relevant experience, at least three years of management experience, reliable transportation, and the ability to pass a background check.

Qualifications

• Bachelor’s degree or a minimum of 8-10 years relevant work experience.
• Three years of increasing management and supervisory experience coordinating residential programs for people with developmental disabilities.
• Verification of a good driving record, valid driver’s license, current auto insurance, and reliable transportation.
• Willingness to participate on a multi-disciplined self-directed team and pass a Criminal Background check.

Responsibilities

• Management of the overall system of supports and services provided to individuals served.
• Personnel management and coordination of service delivery.
• Ensure individuals’ service teams are operating and communicating effectively.
• Financial oversight of program.
• Supervise Team Leaders.
• Ensure Team Leaders have tools to schedule and track appointments and that documentation is completed and filed.
• Ensure regulatory requirements are consistently achieved at all sites.

Responsibilities may include:

• Management of the overall system of supports and services provided to individuals served.
• Personnel management and coordination of service delivery.
• Ensure individuals’ service teams are operating and communicating effectively.
• Financial oversight of program.
• Supervise Team Leaders.
• Ensures that all Team Leaders have tools necessary to schedule and track all relevant appointments (medical, dental, counseling, psychiatric, etc.) and that all documentation is completed, forwarded and filed appropriately.
• Ensures that all regulatory requirements are consistently achieved at all sites.

Requirements for this position include:

• An understanding and acceptance of people with various lifestyles and values
• Bachelor’s degree or a minimum of 8-10 years relevant work experience.
• Three years of increasing management and supervisory experience coordinating residential programs for people with developmental disabilities.
• Eighteen years of age.
• Verification of good driving record, valid driver’s license, current/unexpired auto insurance, and reliable transportation
• Willingness & ability to participate on a multi-disciplined self-directed team
