Evolve Therapy Services in sunny Florida seeks an Occupational Therapy Assistant for an in-house skilled nursing facility. This full-time role requires an active OTA license and a commitment to ongoing professional development.

The position offers a 6-month contract-to-permanent path with relocation assistance and flexible scheduling. You will plan, implement, and document therapy services, collaborate with the interdisciplinary team, and ensure CMS documentation is completed accurately.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice is mandatory.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening and determine best intervention for residents.
• Plan and implement treatment plans in collaboration with supervising therapist.
• Interpret evaluation findings and communicate recommendations to residents and families.
• Establish resident therapy schedules with team members.
• Document and bill occupational therapy services in accordance with plan of care.
• Comply with CMS documentation standards daily.
